Group Health Cooperative of Eau Claire owns the copyright and other intellectual property for the Content of the Site. Neither you nor anyone else has or gets any right, title or interest in and to the Content by simply visiting or using the site. Accounts and Passwords

Members and medical providers have the ability to register for access to the secured area of the Site. To access the secured website, you may need to provide additional information such as organization names, addresses, member identification numbers, group numbers, and tax identification numbers. The purpose of the registration process is to validate members, medical providers, employers, business partners, and agents requesting access to our secured area. Registration also enables us to tailor the information you receive.

We may use the voluntarily provided contact information to notify website users of updates, products, services, or upcoming events. Website users who do not wish to receive such notifications via e-mail can choose to opt out of receiving such information. It is our policy that e-mail messages will contain an "unsubscribe" feature, along with instructions on how to execute the unsubscribe function at any time should you choose to opt out of receiving these notifications.

Your username and password (collectively, the "Password") will allow you to access the Site and certain information about you or your plan. Your Password is the exclusive property and confidential information of Group Health Cooperative of Eau Claire and may be used solely for your individual use of and access to the Site. You are solely responsible for controlling your Passwords and for authorizing, monitoring and controlling access to your account. You are prohibited from distributing, providing or otherwise making available your Password to any third party and are responsible for all activities that occur under your Password. Group Health Cooperative of Eau Claire may terminate any User's right to use the Site at any time. Your right to use the secure portal portion of the Site will end at the close of your membership with Group Health Cooperative of Eau Claire.

Find A Provider

Group Health Cooperative of Eau Claire has made reasonable efforts to validate that its lists of providers displayed are up to date and accurate. Please call Member Services at (715) 552-4300 or (888) 203-7770 prior to scheduling an appointment to verify that your provider continues to be part of the network. Group Health Cooperative of Eau Claire shall not be liable for any losses, damages, or non-covered charges as a result of misusing or misinterpreting the provider locator tool.

Governing Law and Statute of Limitations

The laws of the State of Wisconsin govern these Terms and any cause of action arising under or relating to your use of the Site, without reference to its choice-of-law principles. You agree that the only proper jurisdiction and venue for any dispute with the Company, or in any way relating to your use of the Site, is in the state and federal courts in the State of Wisconsin, U.S.A. You further agree and consent to the exercise of personal jurisdiction in these courts in connection with any dispute involving the Company or its employees, officers, directors, agents and providers. If any provisions of these Terms are determined to be invalid under any applicable statute or rule of law, such provision is to that extent to be deemed omitted, and the balance of the Terms shall remain enforceable.

Before seeking legal recourse for any harm you believe you have suffered arising from and/or related to your use of this Site, you agree to inform us in writing and to give us 30 days to cure the harm before initiating any action. You must initiate any cause of action within one year after the claim has arisen, or you will be barred from pursuing any cause of action. This provision does not supersede any legal rights you may have under federal or state law regarding your policy and/or coverage that is unrelated to use of the Site.
