  2. Click on 'IS MY DOCTOR BOARD CERTIFIED
  3. This will direct you to a website titled "Certification Matters"
  4. In the upper right hand corner click on "IS MY DOCTOR BOARD CERTIFIED"
  5. Fill in required practitioner information and click "FIND MY DOCTOR"
