Provider Model of Care Training

Group Health Cooperative of Eau Claire offers a Dual Special Needs Plan, Cooperative Advantage (HMO D-SNP), that is designed specifically for people who are eligible for both Medicare and full Medicaid benefits (called dual-eligible).

Cooperative Advantage integrates and coordinates benefits available under Original Medicare Parts A and B, Part D prescription drug coverage, and Wisconsin Medicaid benefits to support comprehensive, member‑centered care.

The Centers for Medicare and Medicaid Services (CMS) require Special Needs Plans (SNPs) to ensure that contracted providers who care for SNP members complete annual Model of Care (MOC) training. The Cooperative Advantage Model of Training includes:
