As a member of Group Health Cooperative of Eau Claire, you have the right to elect the Cooperative’s Board of Directors. To be eligible to vote, you must be 18 years of age or older and be a prepaid member of the Cooperative.

To vote, please complete the attached ballot and return it via the United States Postal Service. This postcard does not require a stamp. All ballots must be postmarked by April 6th, 2020.
